How to Make a Claim
- Please fully complete the Accidental Death & Dismemberment Claim Form. Sign a copy of the form, and then mail the form, along with all applicable documents noted below, to the Claims office.

- Coroner's report
- Death Certificate
- Emergency Room report, if applicable
- In the event of death, please include a copy of the insured's Last Will & Testament, indicating Executorship.
- Depending on the claim, additional information may be required.
